Our skilled nursing services are provided by licensed nurses who have received specialized training to provide skilled care to patients with a wide range of medical needs. Our skilled nursing team works with physicians, therapists, and other healthcare professionals to develop a personalized care plan for each patient.

 

Skilled Care includes medication management, wound care, IV therapy, pain management, chronic disease management, and other skilled interventions. Our goal is to help patients achieve optimal health outcomes in the comfort of their own home.

Our skilled nursing services are available for both short-term and long-term care needs. We can provide in-home nursing care following hospitalization, surgery, or a serious illness, as well as ongoing care for patients with chronic conditions.

Our experienced physical therapists will work with each patient to develop a personalized treatment plan based on their unique needs, goals, and medical history. This may include exercises to improve range of motion, strength, balance, and flexibility, as well as pain management strategies and education on proper body mechanics.

 

Our in-home physical therapy program can be highly beneficial for patients who have difficulty traveling to a physical therapy clinic due to mobility issues, transportation limitations, or other factors. By providing care in the comfort of the patient's own home, we can help patients feel more comfortable and confident during their recovery process.

 

In addition to personalized treatment plans, our physical therapists will also provide ongoing education and support to patients and their families on how to maintain and improve their physical function and prevent future injuries.

Our experienced occupational therapists develop personalized treatment plans for each patient, which are based on a thorough evaluation of their physical, cognitive, and emotional abilities. By identifying each patient's strengths and limitations, our occupational therapists create a unique program of interventions to help the patient achieve their specific goals.

 

Our occupational therapy program includes exercises to improve strength, dexterity, and coordination, as well as activities that focus on specific tasks such as bathing, dressing, grooming, and meal preparation. In addition, our occupational therapists educate patients and their families on how to prevent future injuries and maintain independence.

Our speech therapy program is tailored to each patient's individual needs and goals. We start with a thorough assessment of the patient's communication and swallowing abilities, identifying any strengths and limitations. Based on the assessment, our speech-language pathologists develop a personalized treatment plan, which may include exercises to enhance speech production, improve language comprehension, and expand vocabulary. We also provide strategies to help patients cope with cognitive decline and changes in mental function.

 

For patients with swallowing difficulties, our speech therapists also offer interventions to improve swallowing function, such as exercises to strengthen the muscles used in swallowing and strategies to help reduce the risk of aspiration pneumonia.

 

Our speech therapy program can be provided in the comfort of the patient's own home, in a nursing home, or in an outpatient setting. We work closely with patients and their families to provide ongoing support, education, and counseling to ensure the best possible outcomes.

Our Certified Nursing Assistants (CNAs) provide compassionate and skilled care to patients who need help with daily living activities. Our CNAs have completed specialized training and are licensed to provide a range of services, including assisting with personal care, mobility, and meal preparation.

 

In addition to providing hands-on care, our CNAs are trained to monitor and report any changes in the patient's condition. They are also trained to provide emotional support and companionship to patients who may be feeling lonely, scared, or overwhelmed.

 

Our CNAs work as part of a team, communicating with other healthcare providers to ensure that the patient receives comprehensive care. They receive ongoing education and support to stay up-to-date with best practices and continually improve the quality of care they provide.

Our medical social workers provide patients and families with support and guidance during times of illness, hospitalization, or other healthcare-related challenges. They focus on the social, emotional, and financial concerns that patients may experience and work to enhance the patient's quality of life.

 

Our medical social workers are trained to assess the patient's needs and develop a personalized care plan that addresses their unique needs. They may help connect patients with community resources, such as financial assistance programs, support groups, or counseling services. They also help patients navigate complex healthcare systems, which can be especially helpful for those who are new to the healthcare system or those with chronic or complex medical conditions.

 

Our medical social workers also support families and caregivers, providing education, counseling, and referrals to community resources. They may help families with issues related to caregiver stress, bereavement, or communication between family members.

 

Our team of medical social workers work closely with physicians, nurses, and other healthcare providers to ensure that the patient's emotional, social, and financial needs are integrated into the patient's overall care plan. Our transitional care program provides patients with a smooth transition from hospital to home or post-acute care, ensuring that they receive the necessary support and resources to prevent rehospitalization and maximize their recovery.

We also provide placement assistance for those in need of skilled nursing facilities or admissions to independent and assisted living facilities. Additionally, we can coordinate services with geriatric care managers if needed.
